JOB SUMMARY
We are looking for enthusiastic and motivated Health Visitors who are passionate in delivering the Healthy Child Programme. This is an exciting time for individuals looking for new challenges to join our team with many opportunities for development.

MAIN DUTIES AND RESPONSIBILITIES
The specific accountabilities of this role will be flexible and may change to meet the needs of the service users and the organisation as required but will include or be equivalent in nature to those listed below:

• Lead on delivery of the Healthy Child Programme across the community of Barnet.

• To work in partnership with parents, carers, children, young people, Children’s Centres, Children’s Social Care, Primary Care, the voluntary sector, and other members of the multiagency team in delivering public health services that are tailored to meet the needs of individuals and the community.

• To work in collaboration with health partners and other agencies to implement the Barnet Early Help Strategy.

• To ensure that children that are identified as SEND and those with complex health needs and their families receive early intervention and support from the wider multi-agency team.

• To identify vulnerable children and adults at risk, initiating safeguarding and child protection procedures in accordance with the client’s and the Barnet Safeguarding Children Partnership and Safeguarding Adults Board policies and procedures.

• To attend safeguarding statutory meetings, writing case conference reports and Court reports.

• To ensure that the SystmOne electronic records are updated contemporaneously and that records are of a high quality.

• To work flexibly to meet the needs of the service.

• To be innovative and adaptable to different approaches to delivering the service.
Analysis, judgements and decision making

• Assess the physical, developmental, and emotional needs of children within the context of the family and environment.

• Assess the need for further support for disadvantaged families and those in need of safeguarding. Ensure that the analysis and decision-making process is explicit within records.

• Offer and evaluate the Maternal Early Childhood Sustained Home Visiting (MECSH) to those families who meet the criteria.

• Innovate and implement identified target services to reduce health inequalities.

• Access safeguarding supervision, clinical supervision and management supervision as stated within the organisation’s policy documents.

• Manage, analyse, and communicate information relating to health and performance data.

• Contribute to audits and evaluation of service delivery.

Planning and Organising

• Lead on delivering the Healthy Child Programme, in line with the local standard operating guidance.

• To work within the framework of the Baby Friendly Initiative and London Borough of Barnet Infant Feeding Strategy.

• Deliver health promotion relevant to local need.

• Assist and take the lead in project development to meet the service needs and relevant Key Performance Indicators.

• Plan and organise own workload.

• Able to prepare & deliver presentations to a multi-agency audience.

• Able to deliver presentations to different age groups.

• Provide a suitable learning environment and programme for pre/post registration nursing students and new members to the team, through the development of induction programmes, mentorship, and preceptorship.

• Manage own workload, reporting issues to the Locality Leads, when difficulties arise and offer solutions where possible.

Clinical
• Manage a team of skill mix staff to include Staff Nurses, Nursery Nurses and Health Care Assistants.

• Undertake and delegate health assessments of individuals in a wide range of environments and across a broad spectrum of the community.

• Maintain accurate and comprehensive nursing administrative and statistical records, regularly providing the management team with returns and data as required.

• Work in partnership with clients to assess, plan, implement and evaluate programmes of care for individuals and families.

• Support children, young people and their parents/carers and schools to manage children with long term conditions.

• Contribute appropriately to disease outbreak in line with PHE Health Protection Unit guidance.

• Deliver programmes of care in accordance with Standard Operating Guidance and Care Pathways.

• Promote the health, wellbeing and developmental needs of children and young people and their families.

• Promote the safeguarding of children and adults at risk through multi-disciplinary team working and work within the BSCP, SAB and client policies, procedures and guidance for the safeguarding of children.

• Maintain caseload responsibility for children subject to a Child Protection Plan, Child In Need Plan and Looked After Children and delegate appropriately to other members of the team.

• Ensure that confidentiality and the rights of consent relating to young people are followed and that young people are treated with dignity, respect and empathy and in a non-judgemental manner by all members of the team following Department of Health and Education guidelines.

• Work in partnership supporting individuals and families to develop problem solving skills, the fostering of positive relationships and promotion of health.

• Health Visitors who are independent non-medical prescribers must work within the framework of professional accountability.

• Report relevant clinical information/clinical issues to the Locality Lead and Head of Service.

• Maintain knowledge of infection control procedures and implement recommendations as necessary.

• Plan, facilitate and deliver public health promotion opportunities through one to one and group activities in the community.

• Maintain appropriate professional boundaries in the relationships you have with patients and clients.

• Work within a framework that actively supports clinical governance principles managing risk within the team and ensuring quality.

• Support own development by accessing and attending training and development in response to changing needs and local and national guidance.

• Prepare and write referrals and reports for relevant agencies and multi-agency meetings. This includes Safeguarding Case Conferences, Court Reports and multidisciplinary conferences on children with special needs, supported by other members of the team and the Safeguarding Named Nurses as required.

• Complete and maintain contemporaneous child health and family records following NMC guidance

• Ensure personal portfolio is kept up to date in line with the NMC’s revalidation requirements.

The post holder may be required to undertake other duties that may be required from time to time within the general scope of the post. Any such duties should not substantially change the general character of the post. Duties and responsibilities outside of the general scope of this grade of post will be with the consent of the post holder.
The post holder may be required to work flexibly to meet the needs of the local population. This may include some evening and weekend work.
