Description
JOB SUMMARY:
The main purpose of the post will be to provide a high quality research based casework service
to service users. The post holder will be based in the Integrated Adult Community Care Team.
They will provide consultation, support, specialist advice and joint working within the
integrated services to deliver person centred care and support. They will be based within an
identified team, acting in an advisory capacity to members of the multidisciplinary team on
social care issues relating to adults, educating or acting as a mentor as appropriate. They will
also provide training, consultation and supervision as delegated by the Senior Practitioner for
less experienced social workers, support workers and student social workers.

MAIN DUTIES AND RESPONSIBILITIES:
·To provide a social work service to adults living in the Bailiwick of Guernsey. To carry
out assessment using the agreed standardised format. To participate in co-ordinating
services for people wishing to remain within their own homes. To offer support to
people who may be moving into care.
·To work in a flexible way, adapting the service to meet the changing needs of service
users from a wide range of social and cultural backgrounds.
·To work in an identified social work team within the Integrated Adult Community Care
Service.
·To form relationships with service users to formulate an accurate assessment of
complex psychosocial needs of service users, their families and service providers and
assess for services in line with local policy.
·To contribute towards a holistic assessment within the multi-professional specialist
team, recognising that diversity, values and human rights may influence the decisions of
people who use the service.
·To provide complex psychosocial support for service users with terminal illness/life
threatening illnesses and their families, advocating for service users, carers, and families
around complex emotional & practical needs.
·To provide specialist social work advice within the Adult unity Health & Social
Care Team.

CLINICAL:
·To take a key role in responding to referrals to the Adult Social Work team carrying a
caseload of adults in need.
·To respect an individual’s independence and dignity and to offer person-centred support
in making major life decisions and choices.
·To offer support, information & signposting to service users, families and carers on
matters such as housing, benefits and finances, respite care and other community
services, care home placements & third/private sector services.
·Assess service users, carers, and family’s psychosocial concerns taking into account
cultural, spiritual, psychological, social and financial issues and risk evaluation, in
partnership with other members of the multi-professional team as appropriate.
·Assess family’s emotional resilience, coping skills, strengths, and difficulties within this
practise setting, using expert social work skills & discussion with the multi-professional
team.
·Formulate appropriate interventions to meet service user needs in conjunction and liaison
with the multi-professional team, the person’s personal networks and other community
resources.
·Undertake person centred assessments, evaluating & reviewing service user and families’
needs regularly and readjust care plans if necessary.
·Comply with Safeguarding Vulnerable Adults Policy, providing advice to the team
regarding adults at risk, and make referrals as necessary regarding child protection & adult
at risk.
·Be aware of risk as it may relate to service users, carers, children and the safety of health
& social care workers.
·Take responsibility for recognising and addressing the needs of those with mental health
problems, learning disabilities, and other vulnerabilities.
·Produce reports in relation to Long Term Care Benefit and present to the Needs
Assessment Panel in an accurate and timely manner, using fast track where appropriate.
·Identify the need for planned respite for service users in conjunction with other members
of the community team.
·Offer psychosocial consultation to professionals and informal carers involved in caring for
people who are distressed, their families, and carers.
·Provide emotional and psychosocial support to service users and their relatives, using
individual, group, and family techniques with an aim to improve communication within
the family/professional network and help to resolve difficulties. Referring those in need
of formalised counselling to the psychological therapies team.
·Manage an individual caseload, organising and prioritise the workload effectively.

KEY CRITERIA:
ESSENTIAL


1. Proven experience of working in adult services.
2. To possess a full and valid driving licence.
3. To be able to mix easily and communicate with people at all levels.
4. To be enthusiastic and experienced in working in multi-disciplinary teams.
5. To be able to define the contribution of the Social Worker to other professionals within
the team.
6. To be flexible in terms of adaptability to new situations, i.e. working practices / locale /
developing legislation / co-ordinating structures.
7. Proven ability to manage a caseload.
8. Ability to deal efficiently in identifying problems and taking appropriate action.
9. The post demands a high level of autonomy – therefore the ability to prioritise, manage
time effectively and cope when under pressure is essential.

MINIMUM QUALIFICATIONS:
Recognised Social Work qualification E.g. Diploma/Degree in
Social Work or CQSW or Registered with HCPC
