Description
Nursdoc have an exciting opportunity for a contract on the wonderful Island Of Guernsey in The Channel Islands for 3-6 months (maybe extendable)
Nursdoc pay for accommodation and also travel (aeroplane or Ferry (if taking car))
Nursdoc also can pay into your limited company due to different legislation in The Channel Islands.
Candidate must have das two vaccines and won a vehicle (expenses all paid for)

JOB SUMMARY:
1. To assist in assessing, planning, implementing and evaluating programmes of care for patients / clients in a defined caseload.
2. To assist with the management of the caseload.
3. To provide high quality patient care.
4. To assist in the supervision, deployment and teaching of staff and student nurses, acting as a mentor, preceptor / supervisor.
5. To work in the clients / patients’ homes without direct supervision.
6. To work as part of a multi-disciplinary team within an integrated care model.

RELATIONSHIPS:
The post holder will report to the District Nurse Team Leader, will be responsible to the Community Health Care Manager and be accountable to the Service Manager, Community Health and Wellbeing.

MAIN DUTIES AND RESPONSIBILITIES:
CLINICAL RESPONSIBILITIES
1. To assess, develop, implement and evaluate programmes of care for clients / patients in their own homes.
2. To provide prescribed nursing care without supervision.
3. To guide, advise and direct other staff in the delivery of programmes of care.
4. To promote service user involvement in the planning, delivery and evaluation of care respecting their wishes, beliefs and dignity.
5. To empower patients / clients to have greater independence, choice and control over their lives.
6. To liaise and communicate effectively with all members of the multi-disciplinary team and other agencies involved in the care of the client / patient.

TRAINING AND EDUCATION
1. To act as a mentor/preceptor/supervisor to students and other learners.
2. To promote an environment that is conducive to quality learning and assessment.
3. To participate in clinical supervision / reflective practice.
4. Maintain own continuing professional development needs.
5. Develop and maintain competence in the skills related to the needs of the clients / patients at home.
6. To undertake in-house mandatory training.

LEADERSHIP / MANAGERIAL
1. In liaison with the Team Leader, undertake management of the caseload and delegation of work.
2. To be familiar with organisational objectives, policies and procedures related to the work area and the implications this may have for their practice.
3. To act as a professional role model for junior staff, promoting high standards of practice and strong professional values.

COMMUNICATION
1. To communicate effectively with patient/clients and their significant others, taking into account their cultural background and cognitive functioning.
2. To communicate clearly with all members of the multidisciplinary team, demonstrating sensitivity to cultural and language differences.
3. To act as an advocate for patients/clients, significant others and more junior staff members.
4. To ensure written and electronic communication complies with organisational and professional standards.

AUDIT / INFORMATION TECHNOLOGY / RESEARCH ACTIVITY
1. To participate in clinical audits as required to monitor and maintain standards of practice.
2. To utilise the critical incidence reporting system to document actual or potential risks impacting on the quality of patient / client care.
3. To actively promote evidence based practice in care of the client / patient.
4. With training, access organisational electronic information systems relevant to the role.

PROFESSIONAL REQUIREMENTS
1. In accordance with States of Guernsey’s legislation and procedures the Registration of Health and Social Care Professionals (COR001), all nurses and midwives who are States of Guernsey employees, contractors, agency workers or in any other capacity are practising as a nurse or midwife are required to have effective registration with the (UK) Nursing & Midwifery Council (NMC).
2. In order to maintain effective registration with the NMC you must adhere to the Council's requirements for Revalidation.
3. You must proactively manage all aspects of your Revalidation requirements including keeping an up to date portfolio and proactively seeking out opportunities for reflection, continuing education and professional growth. You must be proactive and cooperative in the appraisal and confirmer processes.
4. You must assist colleagues with relevant aspects of their Revalidation. This includes but is not limited to teaching, providing support and opportunities for Facilitated Reflective Practice, participating in professional discussions and providing constructive feedback. Depending on your level of experience and seniority the organisation may also require you to act as a Confirmer.
5. If you are unable to meet any aspect of the Revalidation requirements or this element of your Job Description or you require additional support it is your responsibility to escalate this immediately to an appropriate senior nurse or midwife.

HEALTH AND SAFETY
1. Be able to adapt to unpredictable, hazardous working environments.
2. Be aware of the hazards of working as a lone practitioner. Take reasonable steps to maximise own personal safety and that of others using appropriate risk assessment tools.

OTHER
1. Maintain the welfare and safety of self and others by complying with the Health and Safety at Work (General Ordinance) Guernsey 1987.
